Download Account Forms

  1. Print out the the Account Booklet that's right for you.
  2. Read and fill out, sign and date the forms.
  3. Send the forms in. Please make sure to write CB&S on the first page of the forms and mail to the attention of the CB&S Division. This will facilitate the processing of your account. You can also fax or scan (and email) the completed documents before mailing for quicker processing.
  4. Send funds to your account (by wire transfer or check)

( NOTE: The mininum account size for online trading accounts is $5,000 USD. )

Account Form Booklets

* DOMESTIC: Your personal information will be run through a credit agency to verify your identity. (A copy of your government issued ID and/or of a current bill may be requested.)
* FOREIGN: A copy (of both sides) of your government issued ID or passport and a copy of a current proof of address (heat, electric, land phone or water bill) in the name of each applicant is required.

3. Mail the forms in (you can also Fax or Scan & Email before mailing for quicker processing)

  • please indicate the following on a cover page:
    • The name of our division: CB&S
    • The Trading Platform (if any) you are interested in using

Fax Instructions:
If you wish to expedite the opening of your account, you can fax the forms to us at: 312.902.6191. Be sure to include a cover sheet with your contact information. We will confirm the receipt of the faxed forms and give you further instructions if necessary. Once faxed, please be sure to mail the orignals. Original documentation is required to keep the account open.

4. Send funds to your account

Once you have received confirmation that your account is open and have been assigned an account number, you can wire funds to your account. You can also include a check when you mail in your account forms. Please make checks payable to MF Global. (some types of checks will not be accepted, please see Refco's Check Acceptance Policy for details).

If you need help filling out the forms, just give us a call between 8:00 AM and 4:30 PM (Chicago time), Monday through Friday and we will be glad to help you.
